Agreement-I hereby certify that my child is in normal health and capable of safe participation. I assume all risk(s) and liabilities incidential to the activities of this program. I hereby authorize Stars in Stripes to seek medical treatment for my child in the event that a parent cannot be reached. I understand that refunds are NON-NEGOTIABLE unless written verification is provied by a medical doctor.
